Our Specialty Programs

In addition to skilled nursing and therapy services, Mission Healthcare offers a number of clinical specialty programs and services to assist patients, families and physicians to manage a variety of diseases and conditions in the home environment. *Not all programs are available at all locations
Behavioral Health
Our behavioral health program offers comprehensive psychiatric nursing services to patients with mental health disorders. Our program relies on a dedicated team of behavioral health nurses who provide a comprehensive evaluation and treatment plan using an integrative approach to care. Patients who may benefit from our behavioral health program include: adjustment disorders, mood disorders, depression, mild cognitive impairment, dementia, memory loss, chronic mental illness, and neurological disorders. Our specially trained clinicians work alongside your physician to create an individualized treatment plan to meet your needs.
Fall Prevention & Balance
Aging, physical changes, health conditions and even medications can increase the risk of falls in the home. According to the National Institute on Aging, more than one and three people aged 65 or older fall each year. Our goal is to provide strategies to reduce the risk of falling, so patients can gain the confidence to get back on their feet again. Our specially trained clinicians will evaluate each patient’s home and make recommendations to enhance living safely and independently.
Pulmonary Care (COPD)
Chronic obstructive pulmonary disease, or COPD patients are at risk of hospitalization. Home Health care can help a patient’s symptoms be managed more effectively to improve quality of life and independence. We focus on patient education, self-management, medication management to achieve superior patient outcomes and reduce the need for emergency care. We work alongside the patient’s physician to provide skilled nursing, therapy, home health aides and other services that help the patient manage COPD in the comfort of the home.
Cardiac Care
Patients with heart failure (HF) or congestive heart failure (cHF) can make everyday activities more difficult and cause common symptoms such as fatigue, shortness of breath, and cough. Home Health care can teach patients how to control their medication, diet and lifestyle to improve their cardiac health. Our specially trained clinicians work alongside the patient’s physician to provide skilled nursing, therapy, home health aides and other services that help the patient manage CHF in the comfort of the home.
Home Infusion Therapy
If you need infusion medications, but don’t need to be in a hospital, our home infusion therapy program allows you the freedom and convenience to receive health care services at home. Our specially trained clinicians work alongside the patient’s physician to provide safe and effective home infusion services in the comfort of the home.
Diabetes Management
Diabetes can be a debilitating disease if left untreated or poorly treated. We focus on the education, monitoring, and treatment of patients with Type I and Type II diabetes. Diebetic patients who are eligible for home health can learn how to manage their illness to help reduce risks and maintain a healthy lifestyle.
Wound Care
Our certified nurses focus on training the patient and family to care for wounds to ensure proper healing. Not only do we focus on the treatment and healing of wounds, but also the patient’s overall health. We understand the complex nature of wound treatment and healing. We provide individualized, expert care under a patient’s physician to ensure our patients reach optimal healing.
Memory Care
Older adults with Alzheimer’s or dementia can face serious challenges. As dementia progresses, the cognitive decline can become severe enough to affect a person’s abilities to safely perform activities of daily living. Home health care can be a great benefit to people living with Alzheimer’s and other dementias. Our specially trained clinicians work alongside the patient’s physician to provide skilled nursing, therapy, home health aides and other services that help the patient manage dementia in the comfort of the home.
Orthopedic Rehabilitation
Orthopedic surgeons trust us to help their patients recover quickly after a joint replacement. We assist patients with surgery recovery after hospital discharge in the comfort of the home. Our specially trained therapists help improve mobility, add functionality and provide coordinated care. Our goal is to provide a seamless transition to effectively recover and regain independence in the home.
Journey Care Program
When a patient has a life-limiting illness, our Journey Care Program can bridge the gap between home health and hospice for patients and families. Our program is focused on providing patients with relief from the symptoms, pain and emotional distress of a serious illness. The goal of the program is to improve the quality of life for both the patient and the family.
