Home Health

Home health care helps people remain safely at home and live as independently as possible at home during recovery from a surgery, injury or illness, managing a serious or chronic disease or dealing with multiple diagnoses. Our experienced care team of skilled nurses, medical social workers, therapists and home health aides work with the patient and their physician to create a customized plan of care to improve their quality of life and avoid potential hospitalizations.

Our clinical collaboration and robust technology allows us to deliver the highest quality care possible to our patients in the optimal healing environment, the home. It is our goal to make a positive impact on the lives of our patients, their families and our healthcare partners throughout the continuum of care.



We believe that hospice is more than end-of-life support. It’s about giving every patient the best possible quality of life in a comfortable and caring environment. Our compassionate hospice care team specializes in end-of-life care that provides pain and symptom management for those nearing end of life.

The end-of-life journey begins and ends with love. From our first encounter, we humbly honor the unique physical, psychosocial, emotional, and spiritual needs of each person. With respect and dignity, we give our highest level of service to patients, their families and caregivers, visitors, and to each other.

Our Journey Care Program helps you and your loved ones address a wide range of physical, social, emotional and spiritual needs. It’s appropriate at any age and at any stage in a serious illness, and you can receive it at the same time as curative treatments.

I have worked at Mission now for just over 9 years and it has been a brilliant experience from day one. In hospice work, I know each day what I am called to do, comforting the dying, and supporting families in grief. But it is my teammates—the many compassionate nurses, talented social workers, chaplains, deeply caring home health aides, and more—who really make all the difference, both for our patients and myself, transforming hospice "work" into something more: a caring mission carried out by extraordinary people.
